URSL – Laser Stone Removal from the Ureter

Ureteroscopic Lithotripsy (URSL) is the definitive treatment for stones lodged in the ureter — the tube connecting the kidney to the bladder. A slim ureteroscope is passed through the urethra and bladder into the ureter without any skin incision. Holmium laser energy then fragments the stone into fine dust or small pieces for extraction.

At Chahal Hospital, Dr. Ajay Chahal performs URSL using latest-generation semi-rigid and flexible ureteroscopes. The procedure is highly effective for stones up to 15–20 mm anywhere along the ureter. Stone-free rates exceed 90% for ureteric stones with a single procedure.

URSL is performed under spinal or general anaesthesia, typically takes 30–60 minutes, and most patients are discharged the same day or the next morning. A temporary DJ stent is placed to ensure good ureteric drainage during healing — this is removed in clinic after 2 weeks.

Warning Signs

Signs & Symptoms

Recognize the early signs that indicate you may need medical attention

Renal Colic

Agonising colicky pain from flank to groin — the hallmark of ureteric stone obstruction.

Haematuria

Blood in urine — may be microscopic or visible as pink or red-coloured urine.

Fever with Stone

Fever above 38°C with a known ureteric stone is an emergency requiring urgent drainage.

Nausea & Vomiting

Accompanying severe colic pain, particularly when the stone is impacted.

UTI Symptoms

Burning micturition and frequency from stone-induced infection.

Hydronephrosis

Backpressure kidney swelling from urinary obstruction by the impacted stone.

Understanding the Condition

Common Causes & Risk Factors

01

Kidney Stone Migration

Stones originating in the kidney that migrate into the ureter causing obstruction and colic.

02

Dehydration

Low fluid intake leads to concentrated urine and supersaturation of stone-forming minerals.

03

Diet High in Oxalates

Spinach, nuts, and tea combine with calcium to form calcium oxalate stones.

04

Metabolic Abnormalities

Hypercalciuria, hyperuricosuria, or renal tubular acidosis predispose to recurrent stones.

05

Anatomical Abnormalities

Ureteric stricture, pelvi-ureteric junction (PUJ) obstruction, or horseshoe kidney increase stone risk.

Our Approach

Treatment Options

Advanced minimally invasive techniques for better outcomes and faster recovery

1

X-Ray & CT KUB

Non-contrast CT to confirm stone position, size, and density before planning treatment.

2

Medical Expulsive Therapy

Alpha-blockers and NSAIDs for small stones < 6 mm to facilitate spontaneous passage.

4

DJ Stent Management

Temporary DJ stent for ureteric healing — removed at 2-week clinic review.

Have Questions?

Frequently Asked Questions

No. URSL is performed under anaesthesia. You may have mild discomfort from the DJ stent (urinary frequency, mild bladder spasm) which resolves after stent removal at 2 weeks.

The procedure typically takes 30–60 minutes depending on the number and size of stones. Simple lower ureteric stones may be done faster.

A DJ (Double-J) stent is a thin plastic tube placed inside the ureter to keep it open while swelling from the laser treatment resolves. It typically stays for 2–4 weeks and is removed cystoscopically in clinic.

Yes, bilateral URSL can be performed in a single session if both ureters are obstructed, though this is assessed case by case.

Most people return to desk work within 2–3 days and to physical work within 1–2 weeks after URSL.
